肛門管の解剖と内痔核、外痔核

内痔核（internal hemorrhoid）は歯状線（dentate line）の近位に存在し、円柱上皮に覆われる。一方、外痔核（external hemorrhoid）は歯状線の遠位に存在し、肛門上皮に覆われる。

肛門周囲膿瘍の形成と痔瘻の発生

肛門陰窩（anal crypt）から侵入した細菌が肛門導管（anal duct）を通じて内外括約筋間に存在する肛門腺（anal grand）に初感染巣としての膿瘍を形成し、多彩な方向に炎症が波及する。これらが自潰あるいは切開されて皮膚に開口し、それが瘻管として残存すると痔瘻となる。

裂肛

肛門後方の慢性裂肛。その口側には肥大乳頭（rolled edges）、肛門側には皮垂（sentinel tag）が認められる（裂肛の三徴）。

環状粘膜切除術（PPH）

a：歯状線より2～4cmの位置で全周性にまつり縫いを行う。

b：自動縫合器のアンビルを通し、巾着縫合を行う。この際、女性では膣壁に糸がかかっていないことを確認すること。

c：環状縫合が完了すると痔核は吊り上げられ、固定される。

Abstract/Text: The most important aspect in the diagnosis of hemorrhoidal disease is the exclusion of other, more life-threatening conditions. Hemorrhoidal banding remains the most successful method to manage hemorrhoids in the outpatient clinic. Chronic application of local medications to the perineum may result in dermatologic conditions. 痔瘻癌

25年来の痔瘻があったが、腫瘤の硬結増大とコロイド様の分泌物が出現したため来院。複数の二次口あり、生検にてmucinous adenocarcinomaと診断された。

Seton法

a：歯状線に沿った愛護的な探索で一次口を同定できない場合は、過酸化水素を加えたメチレンブルーを注射することで瘻管の内口を良好に描出できることがある。

b：消息子を一次口から二次口まで通し、皮膚を切開して瘻管を露出し、括約筋をはさみ込む。

c：括約筋が多く分断の必要がある場合は、糸状のゴムを瘻管に通す。

d：手術室でゴム糸を縛り、さらに外来診察時に1～2回縛り直すことで、線維化と漸進的な括約筋の横断を促進する。

裂肛の好発部位

裂肛は大半が後壁側（6時）、次いで前壁側（0時）に発生する。側方発生はまれであるが、肛門部悪性腫瘍、肛門部クローン病、HIV, 梅毒、結核などと関連することがあるので生検を行うべきである。

内痔核と外痔核

内痔核は歯状線より口側に発生する肛門粘膜下組織（anal cushion）の過進展や断裂。出血、脱出が主症状。一方、外痔核は歯状線より外側の肛門上皮部に発生する痔核で疼痛が強い。

裂肛

急性裂肛と慢性裂肛（裂肛、肥大乳頭[肛門ポリープ]、皮垂[skin tag]の三徴がみられる）。

急性および慢性裂肛の模式図。急性裂肛については、肛門管上皮の単純な裂創として左側の小さな図に示した。慢性裂肛では慢性化の徴候として、創縁の翻転、線維化、口側の肥大乳頭、圧痛を伴う肛側の皮垂（skin tag）、内肛門筋の露出などが認められる。

嵌頓痔核

激しい肛門部痛、発熱、悪寒が特徴。痔核の壊死、悪臭の有無、全身所見（発熱、白血球増多、頻脈）によっては緊急痔核切除、デブリードメントを必要とする場合もあるが、通常は入院のうえ、安静、鎮痛薬、排便コントロールの保存的治療により約1週間程度で軽快することが多い。

ゴム輪結紮術

円錐状の先端部を用いてゴム輪を所定の位置に装着したのち、先端部をはずす。肛門鏡を用いて痔核を確認し、それが歯状線より口側で、疼痛を感じないことを確認した後、ファイヤーする。ゴム輪結紮された痔核は通常は数週間の間に脱落する。

痔瘻の分類

痔瘻の4つの主な解剖学的亜型。外括約筋を中心として、瘻管が外括約筋を「貫通（trans）」しているか、外括約筋の「上部（supra）」または「外部（extra）」を走行しているかで分類する。わかりやすくするため、恥骨直腸筋に赤マークをつけている。

Type1：括約筋間痔瘻（intersphincteric）

Type2：括約筋貫通痔瘻（trans-sphincteric）

Type3：括約筋上痔瘻（suprasphincteric）

Type4：括約筋外痔瘻（extrasphincteric）

Goodsallの法則

一次口と二次口の一般的な関係を図示している。図中のAが内口（一次口）である。この法則に従えば、肛門を前後に分ける直線を引いたとき、外口（B）がこの線より前方にある場合には瘻管は直線的に外方に走行し、外口がこの線より後方にある場合には瘻管は曲線的に走行して後交連に内口を有していると予想される。ただし、前方に向かう長い瘻孔はこの法則に当てはまらない。
